PRIMARY PURPOSE

This position is responsible for providing direct support, positive direction and redirection to individuals living with intellectual and developmental disabilities and adults with other disabilities in support of IS Living's mission to Co-create a culture of autonomy and well-being in our communities

Under the guidance of a prescribed service delivery plan aimed at transitioning individuals from dependence to independence, the Therapeutic Mentor works closely with our community health partners and their individualized support teams to insure individuals served receive the best evidenced based care possible.

ESSENTIAL DUTIES AND RESPONSIBILITIES 

  • Assist individuals with activities of daily living
  • Provide verbal and written reports and/or electronic documentation of the outcomes of supervised activities with clients in a timely manner
  • Model positive communication and behavior while working directly with clients
  • Lead therapeutic group activities with clients and other team members
  • Become familiar with the Salem-Keizer Transit system, downtown Salem and Keizer areas, and businesses that clients visit frequently
  • Develop and maintain positive, effective, respectful, and therapeutic relationships with clients and their families
  • Assist in the development of and comply with the Individual Service Plans (ISPs) developed for each client
  • Maintain a clean, safe, professional, and therapeutic environment free from abuse and harassment
  • Report and document both routine and unusual events including completing daily logs, event reports, ISP data sheets, and other necessary records in a concise, understandable and legible manner
  • Maintain open communication and work cooperatively with Program Managers and other team members to assure quality and continuity of support to individuals served
  • Transport clients in a safe manner according to corporate policy and procedure
  • Demonstrate and model the ability to communicate clearly and calmly with clients in behaviorally challenging situations
  • Monitor and document medication counts, manage medication expiration dates and maintain appropriate inventories in an organized manner and in compliance with corporate policy
  • Assist individuals with scheduling appointments, attending, advocating for and assisting with communication efforts during medical appointments
  • Document results of all appointments, secure copy of medical records, and report to Residential Administrator and staff all new or revised medical orders
  • Assist individuals as appropriate in ordering medication, treatments, or supplies per the Residential Administrator
  • Follow-up to assure physician orders are being administered or followed as ordered
  • Monitor and document medication counts, manage medication expiration dates and maintain appropriate inventories in an organized manner and according to corporate policy
  • Document and record all medical and prosthetic equipment, ensuring all medical and prosthetic equipment are in good working order
  • Process orders and maintain an adequate supply of general unexpired medical supplies
  • Collaborate with and assist Medical Case Managers, Psychologists, Nurses, Rehabilitation Therapists and other professionals working with the individual
  • Observe and document individuals' progress to assist in identifying the need for additional services or changes in current plan of care
  • Effectively manage continuous repetitive and potentially aggressive behaviors
  • Maintain a positive relationship with the community including neighbors and local law enforcement
  • Quickly and effectively problem solve individual's needs, redirecting according to their behavioral support plan and utilizing OIS strategies for individuals who become escalated
  • Provide behavioral emergency assistance such as basic first aid when needed
  • Encourage individuals to develop work skills and participate in social, recreational and other therapeutic activities to enhance interpersonal skills and develop social relationships
  • Observe and document individuals' progress to assist in identifying the need for additional services or changes in current plan of care


OTHER DUTIES AND RESPONSIBILITIES

  • Assist the Program Director, Residential Administrator and other team members as requested and assigned
  • Participate in trainings, team meetings and team building experiences as directed by the Program Manager
  • Accept and complete other duties as assigned


MINIMUM QUALIFICATIONS AND EXPERIENCE

  • 18 years of age or older
  • Valid driver's license in good standing
  • Successful clearance of local and or state background checks
  • Negative pre-employment and active employment drug screenings
  • Proficient in speaking, reading and writing in English


PREFERRED QUALIFICATIONS AND EXPERIENCE

  • 2-3 years of experience working with people with cognitive processing skills
  • Minimum of one year experience working in a residential environment with people with cognitive processing deficits
  • Any combination of allied health, psychology, medical, or pre-nursing coursework
